Hip pain is one of the most commonly mislocated complaints we see. People point at their outer hip and say hip, point at their buttock and say hip, and point at their groin and say hip, and those three are genuinely different problems needing different care.
So the first job is location. The hip joint sits deep and forward, closer to your groin than most people expect, and pain arising from the joint itself behaves differently from pain arising from the tendons around it or referred from the spine and pelvis.
What Causes Hip Pain?
These are the things we look for underneath the symptom.
Hip joint restriction and early wear
A stiff hip joint loses rotation first, usually inward rotation. You notice it putting on socks, getting out of a car or sitting cross-legged, and the pain sits in the groin or deep front of the hip.
Gluteal tendon and bursa irritation
The tendons attaching on the outer hip get compressed when you stand with weight shifted or sleep on that side. This is the classic outer hip pain that hurts to lie on and hurts on stairs.
Sacroiliac joint and pelvic imbalance
Buttock pain that people call hip pain is very often the SI joint. It responds to pelvic work, not hip work, which is why the label matters. See our pelvic and SI joint page.
Deep hip muscle overload
The piriformis and deep rotators do a lot of stabilising work. When overloaded they refer pain into the buttock and can irritate the sciatic nerve running beside them.
Where Does It Hurt?
- Groin or deep front of the hip, worse rotating inward
- Outer hip, painful to lie on and sore on stairs
- Buttock ache that worsens when standing on one leg
- Stiffness after sitting that eases as you walk
- Clicking or catching with certain movements
- Difficulty with socks, shoes or getting out of a low car
What Does the Location Tell Us?
This is the single most useful table on this page, because location narrows the cause quickly.

How Do We Find the Cause?
We measure hip rotation and compare sides, test the tendons around the outer hip, and assess the sacroiliac joints and lower back, because the three sources look identical from the outside.
Your first visit is a discovery visit. It includes a full consultation, a set of INSiGHT nervous system scans and, when your history calls for it, digital X-rays taken on site. You leave with a Report of Findings in plain language: what we found, what we would do about it, and how we will measure whether it is working.
What Does Care Look Like?
Once we know the source, the plan is direct and usually combines hands-on work with a small amount of loading.
Hip and pelvic adjusting
Specific adjustments and joint mobilisation to restore the rotation the hip has lost, plus the pelvic work that so often accompanies it.
Soft tissue and myofascial release
Massage therapy through the glutes, deep rotators, hip flexors and lateral hip. This region responds particularly well to hands-on work.
Cold laser for tendon irritation
For outer hip tendon problems, cold laser therapy calms the tissue so strengthening becomes tolerable sooner.
Load changes and strengthening
How you stand, sleep and take stairs, plus two or three specific strengthening movements. Outer hip pain in particular is often a load problem more than a tissue problem.
How Long Does Hip Pain Take to Settle?
Joint restriction and SI-driven pain often change quickly, sometimes within a couple of weeks. Gluteal tendon problems take longer, typically six weeks or more, because the tendon needs progressive loading and the compressive positions that irritated it have to change. Advanced joint arthritis will not reverse, though motion, load tolerance and pain frequently improve, and we are honest about that distinction.
When You Need Something Other Than Chiropractic
Get medical assessment for hip pain after a fall, particularly if you have osteoporosis, an inability to weight bear, pain with fever, night pain that is unrelated to position, unexplained weight loss, or a hip that gives way.
We screen for those findings at every first visit, and we refer the same day when we see them. If a fair trial of care is not moving your objective findings, we will say so and point you to the right practitioner rather than keep you in the schedule.
Questions We Get Asked Most
Is my hip pain arthritis?
It might be, and arthritis on imaging is very common with age and does not always explain the pain. Plenty of people with visible joint wear improve substantially with better mechanics and loading. We aim to improve function rather than argue with an X-ray.
Why does my outer hip hurt when I lie on it?
The tendons and bursa on the outer hip get compressed against the bone when you lie on that side, and often when you lie on the other side with the top knee dropped forward. Pillow position between the knees is one of the quickest changes to make.
Is it my hip or my back?
That is the question the examination answers. Groin pain with lost rotation points to the hip, buttock pain that varies with standing on one leg points to the pelvis, and pain travelling down the leg points to a nerve. Each gets a different plan.
Can chiropractic help before a hip replacement?
Often yes, in the sense of improving how well the rest of you moves and reducing the load on the joint. It does not regrow cartilage. If your joint genuinely needs replacing, we will say so and support you through the process instead.
