Cervicogenic headaches are the most satisfying kind to treat, because the cause is mechanical and it sits somewhere we can reach. The pain usually begins on one side at the base of the skull, spreads up behind the ear and over the eye, and moves with your neck rather than independently of it.

People with this pattern have often been treated for migraine for years without much success. The tell is the neck. If turning your head, reversing the car or a night on the wrong pillow reliably sets it off, the source is very likely the cervical spine.

Why Does the Neck Cause Headaches?

Shared nerve pathways

The upper three cervical nerves converge with the trigeminal system, which supplies the face and head. Your brain cannot always tell the difference between a signal from the top of the neck and one from the head itself, so it refers the pain upward.

Restricted upper cervical joints

When the joints between the skull and the first two vertebrae stop gliding, they become a constant low-grade source of irritation. This is the single most common finding in the pattern.

How Do We Find the Cause?

The examination is the important part here. We look for reduced rotation at specific segments, reproduce the referral pattern from the joints and muscles, and confirm the pain travels the expected route. Where there is a collision in the history, imaging comes first.

Questions We Get Asked Most

How do I know my headaches are coming from my neck?

The strongest clues are one-sided pain that always favours the same side, a start at the base of the skull, reduced neck rotation on that side, and a reliable relationship with neck position. If we can reproduce your headache from the neck during the examination, that is fairly conclusive.

Will I need ongoing care forever?

No. Most people move from a corrective phase to occasional maintenance, and plenty stop altogether. How long you continue is your decision, and we will always tell you when we think the corrective work is done.

Can my pillow be causing this?

It can certainly be maintaining it. Seven hours a night in a position that closes down one side of your neck is a meaningful load. We will look at your sleeping position as part of care, because it is one of the cheapest things to change.

What if I also get migraines?
