Mid-back pain is the one people apologise for. It is not dramatic like a disc, it does not stop you walking, and it usually gets described as a knot someone needs to dig out. Then it comes back within the week, in exactly the same spot, and no amount of digging changes that.
The reason is simple. The middle of your back is where twelve vertebrae meet twenty-four rib joints, and if any one of those has stopped gliding, the muscles around it stay switched on to protect it. Releasing the muscle without restoring the joint is why the relief never lasts.
What Causes Mid-Back Pain?
These are the things we look for underneath the symptom.
Thoracic joint restriction
The thoracic spine is built for rotation. Days spent facing forward at a desk or a steering wheel remove that rotation, segments stiffen, and the surrounding muscles take over as stabilisers. The result is that familiar band of tension between the shoulder blades.
A rib that has stopped moving
Every rib forms a joint with the spine and moves each time you breathe. When one loses that motion you get a sharp, catching pain with a deep breath, a cough or a sneeze, often mistaken for something more alarming.
Breathing mechanics
If the diaphragm is not doing its share, the neck and upper chest muscles pick up the work. That pattern shortens the front of the chest, pulls the shoulders forward and loads the mid-back all day long.
Sympathetic nervous system load
The nerves that run your stress response exit the spine through the thoracic region. Chronic stress and mid-back tightness feed each other, which is why this area often flares in the busiest weeks of the year.

How Do We Find the Cause?
We check segment by segment for which joints and ribs have lost motion, then compare that against what your muscles are doing. Surface EMG is useful here because it shows the compensating pattern rather than just the sore spot.

What Does Care for Mid-Back Pain Look Like?
The aim is to restore motion first, then take the load off the muscles that were covering for it.
Thoracic and rib adjusting
Specific, low-amplitude adjustments to the restricted segment and its rib. Most people describe immediate relief in how freely they can breathe, before the soreness itself changes.
Soft tissue and myofascial release
Massage therapy through the mid-back, chest and lats releases the muscles that have been acting as splints, which is what stops the catch from returning.
Breathing retraining
Two or three minutes of diaphragm work a day is often what holds the correction. We teach it in the room rather than handing you a sheet you will not open.
Cold laser for stubborn spots
Where a rib joint stays irritated, cold laser therapy reduces inflammation so the adjustment holds longer.
How Quickly Does Mid-Back Pain Respond?
Mid-back complaints are often the fastest thing we see to change, because the segment usually just needs its motion back. Many people notice a difference in breathing depth on the first visit. Long-standing postural patterns take longer, since the desk, the phone and the car seat that built them are still there. That part is a habit project as much as a treatment one.
When You Need Something Other Than Chiropractic
Mid-back pain deserves medical assessment first if it came with a significant fall or collision, if you have osteoporosis and the pain began without a clear cause, if it is constant and unrelated to movement, or if it travels around the chest with fever, shortness of breath or a rash.
We screen for those findings at every first visit, and we refer the same day when we see them. If a fair trial of care is not moving your objective findings, we will say so and point you to the right practitioner rather than keep you in the schedule.
Questions We Get Asked Most
Why does my mid-back pain keep coming back after a massage?
Because the muscle was doing a job. If the joint or rib underneath it has not regained motion, the muscle tightens again to protect it. Combining soft tissue work with a specific adjustment is what breaks that cycle.
Can mid-back pain make it hard to breathe?
A restricted rib joint can absolutely make a deep breath feel pinched or shallow. It is mechanical rather than respiratory, and it usually eases quickly once the rib moves again. Breathlessness with chest pain or dizziness is a different matter and needs urgent medical assessment.
Is my posture the reason for this?
Posture is a load, not a verdict. Hours facing forward remove the rotation your thoracic spine needs, and pain follows the load. We restore the motion and then work on the two or three habits that keep taking it away.
Does it help with the tension I carry from stress?
Often, yes. The thoracic spine is where the sympathetic nervous system leaves the spinal cord, so this region tends to hold stress. We track that objectively with INSiGHT scans rather than guessing at it.
