The short answerThe knee is a hinge caught between the hip and the foot, and it usually hurts because one of those two is not doing its job. Care that only treats the knee tends to give short-lived relief.

Knee pain is rarely a knee problem alone. The knee is a hinge, and it does what the hip above it and the foot below it allow. When the hip cannot control rotation or the foot collapses inward, the knee absorbs the difference with every step.

That is why so many people have iced, braced and rested a knee that keeps hurting. The tissue that hurts is a symptom of mechanics happening somewhere else, and until those change the knee keeps taking the load.

What Causes Knee Pain?

These are the things we look for underneath the symptom.

Poor hip control

The glutes control how much your thigh rotates inward when you take a step. When they are weak or poorly timed, the knee drifts inward under load, which is the most common driver of pain around the kneecap.

Foot and ankle mechanics

A foot that rolls inward excessively rotates the shin and changes how the kneecap tracks. Stiff ankles have the same effect from the other direction. See our ankle mechanics page.

Kneecap tracking problems

The kneecap glides in a groove. When the surrounding muscles pull unevenly it tracks off-centre, producing pain on stairs, after sitting and when squatting.

Tissue overload and degeneration

Meniscus irritation, tendon problems and joint wear all cause pain. Even then, the mechanics above and below determine how much load that tissue takes every day.

What Are the Common Patterns?

  • Pain around or behind the kneecap, worse on stairs
  • Aching after sitting for a while, easing once you move
  • Pain with squatting, kneeling or getting up from low chairs
  • Pain on the inner or outer side of the joint
  • Swelling after activity
  • A sense of the knee giving way or catching

What Kind of Knee Pain Is It?

These four patterns cover most of what we see and each points to a different plan.

What you noticeMost likely sourceWhat care involves
Front of knee, worse on stairs and after sittingKneecap tracking and hip controlHip strengthening, soft tissue, foot mechanics
Inner joint line, worse twisting or squattingMeniscus irritationLoad management, laser, graded return, imaging if needed
Outer knee, worse running or downhillLateral tissue overloadHip control work, soft tissue release
Global stiffness and morning ache with ageJoint wearMotion, loading and load reduction from above and below

How Do We Find the Cause?

We watch you squat, step and balance on one leg, then test hip strength and ankle motion, because the knee is usually the victim rather than the culprit. We test the knee structures themselves as well.

Your first visit is a discovery visit. It includes a full consultation, a set of INSiGHT nervous system scans and, when your history calls for it, digital X-rays taken on site. You leave with a Report of Findings in plain language: what we found, what we would do about it, and how we will measure whether it is working.

What Does Care Involve?

Fix the mechanics above and below, calm the irritated tissue, then load it correctly.

Adjusting the hip, knee and ankle

Restoring motion at the joints that are limiting things, with specific adjustments and mobilisation rather than treating the knee alone.

Soft tissue release

Massage therapy through the quadriceps, hamstrings, calves and lateral thigh to change the pull on the kneecap.

Cold laser for irritated tissue

Cold laser therapy for tendon and joint line irritation, which makes the loading phase considerably more comfortable.

Hip and foot strengthening

A small number of specific movements aimed at hip control. This is what stops the pain coming back once it settles.

How Long Does It Take?

Kneecap-related pain often improves within two to four weeks once hip control changes, though the strengthening needs to continue longer to hold it. Meniscus and tendon irritation take longer and require graded loading. Established joint wear does not reverse, but load tolerance, stiffness and daily pain often improve, and that is usually what people actually want.

When You Need Something Other Than Chiropractic

Seek medical assessment for a knee that locks and will not straighten, significant swelling immediately after an injury, an inability to weight bear, a hot swollen knee with fever, or a knee that gives way repeatedly.

We screen for those findings at every first visit, and we refer the same day when we see them. If a fair trial of care is not moving your objective findings, we will say so and point you to the right practitioner rather than keep you in the schedule.

Questions We Get Asked Most

Can chiropractic help knee pain?

Yes, particularly when hip and ankle mechanics are driving it, which is the majority of non-traumatic knee pain. We treat the knee too, but the reason care holds is usually what we changed above and below it.

Do I need an MRI?

Usually not to start. Examination findings guide the plan, and imaging matters if the knee locks, swells significantly or fails to respond to conservative care. We refer when that is the picture.

Should I stop running?

Rarely completely. More often we adjust volume, surface and cadence while we change the mechanics, because complete rest tends to deload the very tissues that need to get stronger.

My X-ray shows arthritis. Is there any point?

Frequently, yes. Imaging findings and symptoms correlate loosely, and reducing the load reaching a worn joint often reduces pain meaningfully. We will be straight with you about what is realistic.

Knee still hurting after months of icing it?

It is probably time to look at the hip and the foot. Come in and we will assess the whole chain rather than just the sore part.
