Sciatica is the pain that makes people cancel plans. Sitting through a meal becomes unbearable, driving to Canton feels like a project, and the leg develops a mind of its own with burning, numbness or pins and needles that arrive without warning.
Here is the part that matters. The word describes a symptom along the path of the sciatic nerve, which runs from the lower spine through the buttock and down the leg. What is irritating that nerve varies from person to person, and treating every leg pain identically is exactly why so many people have tried care that did not work.
What Causes Sciatica?
Three sources account for most cases, and telling them apart changes everything about the plan.

The most common cause. When disc material pushes into the outer wall it can contact the nerve root as it leaves the spine. This version is usually worse with sitting, coughing, sneezing or bending forward, and often improves when you stand and walk.
SI joint and pelvic dysfunction
The sacroiliac joints sit either side of the tailbone. When one stops moving properly it refers pain into the buttock and down the back of the thigh, typically stopping above the knee. Rolling over in bed and standing on one leg are the usual giveaways.
Deep hip muscle involvement
The sciatic nerve passes very close to the piriformis and the deep hip rotators. When those muscles are tight or overworked they can compress the nerve directly, which is why some sciatica changes with hip position rather than spinal position.
A nervous system that has become sensitised
When pain has been present for months, the system that carries it turns its own volume up. Care that ignores this is why some people improve mechanically but still hurt.
What Does Sciatica Feel Like?
- Pain that travels from the low back or buttock into the leg

- Numbness, tingling or pins and needles in the leg or foot
- Worse with sitting, driving, coughing or sneezing

How Do We Find the Cause?
We use position testing, nerve tension testing and your history to work out which structure is involved, then confirm with scans and imaging where indicated. That distinction is the whole point of the first visit.
Your first visit is a discovery visit. It includes a full consultation, a set of INSiGHT nervous system scans and, when your history calls for it, digital X-rays taken on site. You leave with a Report of Findings in plain language: what we found, what we would do about it, and how we will measure whether it is working.
What Does Sciatica Care Look Like?
Once we know the source, the plan is straightforward and usually combines two or three of these.
Non-surgical spinal decompression
For disc-related sciatica, decompression therapy lowers pressure inside the disc so fluid returns and the nerve root gets room. It is comfortable, and for the right candidate it is a genuine alternative to injections or surgery.
Specific adjustments
Adjustments restore motion to the lumbar spine and pelvis. In an acute flare we usually start with low-force instrument or drop table technique rather than manual work.
Cold laser therapy
Cold laser targets the inflammation around an irritated nerve root, which is often what turns the burning quality of the pain down.
Deep tissue and myofascial release
Where the deep hip muscles are involved, massage therapy is not optional. Releasing the piriformis and hip rotators takes pressure off the nerve directly.
How Long Does Sciatica Take to Improve?
Nerve pain typically changes in a specific order. The distance the pain travels shrinks first, so it stops at the knee instead of the foot, then the intensity drops, then the numbness fades last. Numbness and tingling are usually the slowest to resolve because nerve tissue recovers slowly. We track the distance the symptoms travel visit by visit, which is a far better measure than a pain score.
When You Need Something Other Than Chiropractic
Sciatica needs same-day medical assessment if you have numbness through the groin or saddle area, difficulty starting or controlling urination, loss of bowel control, or weakness that is getting worse rather than better, such as a foot that catches when you walk.
We screen for those findings at every first visit, and we refer the same day when we see them. If a fair trial of care is not moving your objective findings, we will say so and point you to the right practitioner rather than keep you in the schedule.
Questions We Get Asked Most
Can a chiropractor fix sciatica?
We can often resolve the mechanical cause of it, which for most people is what sciatica is. The honest answer depends on the source, how long it has been there and whether nerve tissue has been compressed for a long time. You will get a specific answer at your Report of Findings rather than a promise on day one.
Should I rest or keep moving?
Mostly keep moving, within comfort. Prolonged rest stiffens the joints and does nothing for disc nutrition. Walking is usually well tolerated even when sitting is not. What we ask you to avoid is the specific position that reliably reproduces your pain.
Is spinal decompression better than surgery?
It is a reasonable thing to try first for the right candidate, since it is non-surgical and reversible. Certain findings rule it out, including some fractures, advanced osteoporosis and specific post-surgical hardware, which is what the examination screens for.
Why does my sciatica come back?
